1. Basic Information Fields

  1. Name Field: Collect the respondent's name for personalized communication. This can be optional if you prefer anonymous feedback.

  2. Email Address Field: The email address field allows you to follow up with respondents for more details or to thank them for their feedback. This can be optional, depending on your preference for anonymous submissions.

2. Multiple Choice or Dropdown Questions

How to Use the Feedback Form Template on Your Website

  1. 1. Accessing the template: To begin, open the Likeable Forms form builder and navigate to the Templates section in the settings menu.

  2. 2. Import the template: Hover over the Feedback Form Template and click Import to add it to your form collection. This will load the basic structure of your feedback form, ready for customization.

  3. 3. Customize Your Fields: Edit the Rating System, Multiple Choice/Dropdown Questions, and Open-Ended Textarea to match the type of feedback you’re seeking. You can also adjust the Net Promoter Score (NPS) question or add any other specific fields relevant to your feedback collection.

  4. 4. Add or Remove Questions: Feel free to add new questions or remove any that don’t apply. You might want to include more questions based on your industry or the type of feedback you need—be it product feedback, service satisfaction, or general customer experience.

  5. 5. Set Up Notifications: Configure email notifications to alert you or your team when new feedback submissions come in. You can also set up automatic confirmation emails to thank users for their responses.

  6. 6. Publish the Form: Once you’ve customized your form, save and publish it. You can embed the form on your website’s feedback page, on your blog, or in post-purchase emails to gather insights from your customers.
